Skilled Nursing
Skilled Nursing is the backbone of home health care. A nurse will be a liaison between the patient and other healthcare professionals. Specific responsibilities include:
- Direct care for patients with diabetes, heart disease, bone fracture, renal failure, past strokes, and more
- Help patients and caregivers understand different disease processes and possible complications
- Provide instruction on many restorative and preventative measures
Physical Therapy
Registered Physical Therapists will evaluate the patient and devise a program according to the physician’s plan of treatment. Services include:
- Restorative muscle function training
- Teaching of transfer techniques in all areas
- Re-training in walking, stair climbing, etc.
Occupational Therapy
A Registered Occupational Therapist will introduce activities designed to achieve optimum level of rehabilitation to promote independence and safety at home.
The therapist will evaluate and establish a program using adaptive devices and therapeutic regimen, geared to assist the patient in attaining maximum functional independence.
Speech Therapy
Speech Therapists provide valuable services to patients and their families. Their service includes evaluating speech, language, and communication dysfunction, while reporting findings and recommendations to the physician.
Labs at Home
Lab work has never been more convenient; we bring all the necessary supplies to do the collection at home. Our lab technicians can perform a full spectrum of diagnostic tests, such as blood draws, liver function tests, CBC, PT/INR, Hemoglobin A1C, and more.
Home Health Aide
Aides provide personal care, assisting with activities of daily living, nutrition, and homemaking tasks. Our Home Health Aides are supervised by Registered Nurses to ensure compliance with the established plan of care.
They assist the Registered Nurse in monitoring vital signs, identifying changes in the patient’s condition, and flagging abnormal signs and symptoms.
Medical Social Services
Following the clinical model of social work, we provide short term counseling and assistance in coping with lifestyle changes created by illness, and a link to various community resources.
The Social Worker contributes to the care of the patient by creating an environment that promotes recovery.
